Our Care Follows You Home
Our care doesn't end when you are discharged from our Center. We work with each older adult, his or her family, and physician to develop a comprehensive discharge plan. If needed, we can also act as a liaison between the family, physician, and other care facilities to ensure the patient's needs and goals are met.

We aggressively evaluate medical conditions that may be contributing to depression or cognitive difficulties. We work with each person's current abilities and strengths to stabilize his or her condition, establish new behaviors, and implement a treatment plan that promotes the best recovery. |
